Sacred Heart Visiting Nurses is a nonprofit, hospital-based home care agency, which serves patients of all ages in Berks, Lehigh, and Northampton Counties. Full-service capabilities are provided to meet the patient’s total home care needs. Sacred Heart Visiting Nurses provides services such as skilled nursing, physical therapy, occupational therapy, speech therapy, social services and home health aide services. Sacred Heart Hospice is a certified provider of the Medicare Hospice Benefit and is approved by numerous insurance plans. The goal of Hospice is to enable terminally ill patients, young and old, to live life to the fullest in their homes among family and friends. The multidisciplinary team includes physicians, nurses, therapists, medical social workers, clergy, other counselors and volunteers. This team develops an individualized plan of care that focuses on physical, emotional and spiritual comfort during the final stages of illness and bereavement.
